2026 Blended Medical Plan

American Senior Communities offers the Blended Medical Plan as one of the two medical plans offered in 2026.

The Blended Plan will be administered by AmeriBen and has two components and two medical ID cards. One card with the Anthem logo and a second card without the Anthem logo. 

The Anthem network coverage is for physician and ancillary services. The ID card with the Anthem logo will be used for these services.

Reference Based Pricing will be used for all facility services. The Imagine 360 card without the Anthem logo will be used for these services.

The prescription drug benefit is offered through True Rx and is a component of the medical plan.

This benefit is available for all employees hired and working in full-time status (minimum of 30 hours a week). Premium deductions are taken pre-tax.
